| • Tax documentation to support your engagement with our company, according to your country requirements. |
| • If you will be receiving payment from our entity in the United States and you are based in the United States, you will need to provide your current W9 form. |
| • If you will be receiving payment from our entity in the United States and you are based outside of the United States, and your engagement with us is as a company, you will need to provide your W8 BEN E form and Withholding Tax Form. |
| • If you will be receiving payment from our entity in the United States and you are an individual based outside of the United States, you will need to provide your W8 BEN form and Withholding Tax Form. |
| • If you will be receiving payment from our entity in Puerto Rico but are not based in Puerto Rico, please complete and upload this Withholding Tax Form. |
| • If you will be receiving payment from our entity in Canada but are not based in Canada, please complete and upload this Withholding Tax Form. |
| • If you will be receiving payment from our entity in Poland but are not based in Poland, please complete and upload this Withholding Tax Form. |
| • If your engagement with our company is exempt from Withholding Tax, please provide exemption certificate or details. |
